Mijëra të punësuar në Shqipëri paguajnë çdo muaj kontributet e detyrueshme për sigurimet shëndetësore. Megjithatë, në praktikë, këto kontribute u japin të drejtën të marrin shërbim vetëm në institucionet publike, ndërsa në spitalet private, edhe për të njëjtin shërbim, duhet të paguajnë nga xhepi.
