How will salaries for healthcare workers change? - From seniority to remote service and emergencies

The Council of Ministers has approved a package of changes to the salary and financial treatment scheme for healthcare system employees, changing the basic salary, seniority allowances, emergencies and critical services, as well as compensation for working away from home.
One of the main changes is related to the reformulation of the seniority allowance, which will now be calculated on the salary of the position and will undergo a progressive increase according to years of service. Specifically, healthcare system employees will benefit from an allowance of 0.6% for each year of work for the first 10 years, 0.8% for 20 years and 1% for 30 years of service. This formula aims to assess experience and sustainability in the system.
The decision also provides for special monthly allowances for critical and busiest hospital services. Doctors working in emergency, resuscitation, hemodynamics, in cerebral accident units and in neonatology will benefit from an allowance of 18 thousand lek per month. Meanwhile, nurses, imaging and laboratory technicians in these services, as well as doctors and pre-hospital emergency medical nurses at the National Emergency Medical Center, will receive a monthly supplement of 12 thousand lek.
Another important element of the decision is related to the payments for the emergency and on-call service, which have been increased and differentiated according to working hours and days of the week. For 24-hour services, payments range from 2,700 lek on weekdays, up to 3,200 lek on Sundays and official holidays, while special rates are also provided for 16- and 12-hour systems.
The decision also reconfirms the monthly compensation for doctors and technical personnel working away from their place of residence, a measure that aims to cover the difficulties of providing service in remote areas or with a shortage of health personnel.
Within the framework of the general salary policy, the new scheme also includes the indexation of 2.5% of salaries of positions in the health system, as part of the annual salary adjustment in the public sector for 2026.
The changes also affect the organizational aspects of state institutions, giving the heads the opportunity to contract up to three external advisors, Albanian or foreign, with a monthly payment of up to 112,750 lek after approval by the Ministry of Health.
The new decision extends the financial effects from January 1, 2026 and applies to all public health institutions, including those under the Ministry of Health, the health structures of the Armed Forces, the prison system and the State Police.
